Navigating the Dry Eye Disease Desert – by Dr. Cameron Steflik

Welcome, fellow wanderers of the ocular desert! It’s a parched land where blinking feels like treading sand dunes and eye drops are your canteens. That’s right, we’re diving into the fascinating (and often frustrating) world of Dry Eye Disease! So, grab your virtual camel and let’s explore this condition. 

What Exactly is Dry Eye Disease?

Picture this: you’re staring at your computer screen, your favorite book, or perhaps the majestic outdoors. Suddenly, your eyes start to sting, itch, or feel gritty. Contrary to what the name might suggest, it’s not just about a lack of tears. It’s a complex condition where your eyes might be flooded with tears but lack the proper balance of oils, water, mucus, or nutrients that create the perfect tear cocktail for eye comfort.

Journey Through the Symptoms

Any one of the following symptoms, or combination thereof, can be a sign of Dry Eye Disease:

Sandy Sensation: It feels like your eyes are tiny sandboxes filled with irritation.

Red Eyes: Your eyes look like they’ve partied all night, and the result? Bloodshot eyes that might take some explaining to coworkers.

Blurry Boundaries: Suddenly, you’re Monet without the talent. 

 

Watery Mirage: Counterintuitive as it may sound, dry eyes can provoke a river of tears. It’s like your eyes are trying to turn the desert into a waterpark.

 

 

 

Navigating to the Oasis of Relief

Not to worry! There are options to relieve these irritating symptoms, including:

Hydration: Stay hydrated! Drinking enough water helps your overall health and can also maintain the moisture in your eyes.

Blinking Pilates: Be conscious of your blinking. Staring at screens for extended periods can lead to reduced blinking, contributing to dryness.

Omega-3s: Including foods rich in omega-3 fatty acids like salmon, flaxseeds, and walnuts in your diet can boost tear production and eye comfort. Consider supplementation. 

Artificial Tears: Just like a cold beverage at the beach, your eyes deserve some relief. 

Humidify the Desert Air: Adding a humidifier to your living or working space can infuse much-needed moisture into the air and your eyes.

Why Are More People Wearing Glasses These Days?

More and more children are needing glasses at younger ages. In the U.S. in the 1970s, a quarter of the population was nearsighted. In the early 2000s, 42 percent of the population was nearsighted. In East and Southeast Asia, where this shift is most dramatic, the proportion of teenagers and young adults with myopia has jumped from roughly a quarter to more than 80 percent in just over half a century. More recent surveys have not been performed in the U.S., but from personal experience and after consulting with other optometrists, it is clear there are more nearsighted kids today than previous generations. 

Why is This Happening?

This sudden increase in nearsightedness is baffling since poor vision was originally thought to be impacted primarily by genetics. Genetics is a factor, but there’s more to it. Just look around and you’ll see countless kids absorbed in phones, tablets, and laptops. Who would have thought that looking at something 4 inches from your face for hours at a time could be bad for you? Our ancestors needed excellent distance vision to survive, while most of our daily activities involve looking at screens indoors. Perhaps the motherly admonition to not sit so close to the TV had some merit.

Another theory involves the reduction of time spent outdoors. This theory suggests that natural light exposure aids developmental modulators that form the eyeball. Children today spend far more time indoors than previous generations. Both theories are 2 sides of the same coin. Whichever theory is true, you can draw the same practical conclusion for children’s visual development: less time hunched over a screen, and more time playing outside. 

The majority of babies are born farsighted. Our eyes start slightly too short, and they grow in childhood to the right length, then stop. This process has been finely calibrated over millions of years of evolution. But when the environmental signals don’t match what the eye has evolved to expect the eye just keeps growing.

So Why Does This Matter?

So what if more children need to wear glasses? Glasses, contact lenses, and laser surgeries all help people see better, but they do not treat the underlying problem. Patients with severe myopia have elongated eyeballs- they look more like olives than spheres. This stretching of the eye increases risks of developing ocular disease including retinal detachments and glaucoma later in life which can lead to debilitating blindness. Nearsightedness also can lead to a reduced quality of life, affecting daily activities such as driving, sports, and social interactions.

Can We Do Anything? 

Once the eye has grown it doesn’t ungrow. Prevention is key. Children need more time outdoors, but unfortunately this likely is not enough due to the increasing demands on near vision. Special contact lenses called multifocal lenses alter the way light enters the eye.This sends the signal for the eye to stop lengthening.  

In November 2019, the FDA green-lighted the first-and currently only-treatment specifically designed to slow the progression of myopia, a soft contact lens from CooperVision called MiSight. It has been shown to slow progression up to 50%.

For more information on myopia control- the American Optometric Association’s evidence-based committee published a report advising its members on how to use myopia control.

Transition Into Summer!

Summer has arrived like a kid on the morning of a beach trip, eager for mom and dad to get out of bed. The water is calling to us, and we hope everyone gets out to swim, boat or float. We know we’re planning on it!

As much as the sun feels amazing on your skin, you can definitely have too much of a good thing. We all know the sun can be dangerous for your skin, but it’s just as harmful to your eyes. Up to 10% of all skin cancer occurs around your eyelids. This delicate skin needs protection too, though not of the sunscreen kind. No, no, no sunscreen in or around your eyes! Protecting the areas around the eyes is just as important as protecting the inner eye. Along with this, the majority of us will develop cataracts at some point in our life, and UV rays are one of the main causes. Just as important is HEV (or Blue Light) protection, specifically from the sun, to make sure we’re protecting your retinas and maculas. You know, all those parts of the eye that you see with!

There’s a great way to make sure your eyes are protected no matter what the situation! All you need are Transition™ auto-darkening lenses!

Did you recoil at the thought of a darkening lens? Don’t worry, you’re not alone, and we definitely understand why you might feel that way. This technology has come a long way! Where they may have taken waaaay too long to go back to clear in the past, they now do so in 60 to 90 seconds flat on average! Speaking of clear, you don’t have to worry about any kind of tinting on the lenses when indoors, in fact you wouldn’t know a Transition™ lens darkens unless you walked outside.

Transitions™ provide much more protection than the sunglasses you get from Target or any drugstore . They protect from  UVA and UVB , as well as up to 85% blue light protection. When they’re clear, they give you the best protection against the blue light from your phone and computer screen that you can get.

Did we mention that they look amazing too? Because they absolutely do! Gone are the days where the tint had to be either gray or brown. Now you can choose from sapphire, amethyst, emerald, and amber! There’s a color for every amazing frame you choose!!

July is Dry Eye Month!

July is Dry Eye Awareness Month!

In recognition of Dry Eye Month, Dr. Greyson Ramos introduces our patients to some of the treatments and care that we offer for Dry Eye Disease at Envision.
What causes Dry Eye Disease?

Dry Eye Disease, or DED, is caused by many environmental and lifestyle factors. Symptoms occur when the ocular surface and structures begin to breakdown from a lack of lubrication. There are two sources of lubrication: the aqueous (water) tear layer and the lipid (fatty) tear layer. In DED, it is possible for both sources to be impacted, although the fatty layer is more commonly affected. Many experience excessive watering, dry, and gritty-feeling eyes. The best solutions is not only treating the symptoms, but also increasing overall health and solving the cause of DED. When diagnosed, our doctors begin by educating patients on small daily changes. Drinking sufficient amounts of water and blinking exercises can help increase healthy, natural tear production. After patients incorporate these changes into their routines, we begin treatment of DED with ocular therapeutics.

Level 1:

The first level of DED treatment begins with artificial tears, eyelash cleaning, and warm eye compress masks. These solutions encourage a clean, well-lubricated ocular surface free of pollen, old make-up, etc. If after three months or sooner “Level 1” is not resolving symptoms, we move the patient to “Level 2”.

Level 2:

Second level treatment incorporates more robust artificial tears designed to treat the cause of fatty tear film deficiency. We also introduce an “Intense Pulsed Light (IPL)” therapy treatment at this stage. The IPL uses purposely selected light intensity to cause a decrease of leaky, inflammatory blood vessels that develop in DED. IPL therapy assists in kickstarting the tear film production factory, and has an added benefit of relaxing wrinkles, removing acne scars, and eliminating sun/age spots! If “Level 2” does not resolve symptoms, we begin “Level 3” treatment.

Level 3:

Level 3 incorporates specialty contact lens materials embedded with stem cells to encourage new production of damaged eye tissue, and eye drops that are made from the affected patient’s own blood as a therapeutic remedy! Since most patients will receive complete symptom relief from a “Level 2” treatment, this level is reserved for more advanced cases of DED.

New Policy for Special Testing Appointments

Hello Wonderful Patients!

We wanted to update our patients on our NEW No-Show/Cancellation Policy for Special Testing appointments. This new policy is effective immediately.

If you are scheduled for this appointment type, your doctor has ordered it for you to help identify and/or monitor an eye health condition. These include Glaucoma, Age-Related Macular Degeneration and more.

We work to maintain a high-quality scheduling system and availability. The time allocated for this appointment is scheduled specifically for you. When an appointment is cancelled last minute, that appointment opportunity is lost for another patient who may also need this level of care.

We strive to provide the best care for your eyes. Confirming and attending your appointments is very important for your precious eye health!

If you need to make changes or cancel your appointment, we require at least 24 hours notice to avoid the $100 cancellation fee.

 

Please keep in mind, this same $100 fee also applies to our yearly comprehensive eye health exams.

Spectera Out-Of-Network as of May 1st!

Hello Dear Patients!

In order to maintain our high standard of care, Envision Eyecare will no longer be in network with Spectera Vision beginning May 1st, 2021.

Our Doctors feel that participating with the Spectera Vision Plan limits the level of care that Dr. Golson and Dr. Bryant can provide for you.

Each Spectera plan is different, and may reimburse participating patients for out-of-network rates. You will need to check with Spectera directly to ask if this option is available for you. We are happy to provide you with the necessary documents for filing with them as we would love the opportunity to continue with your care!

As the Integrator for successful guidance, team collaboration and positive patient results over the last 6 years, I’ve had the opportunity to lead this organization and team through many changes, challenges, celebrations and creative advancement. Every aspect of how we engage with each other, our patients and our physical office space has adapted to life during a pandemic and it was A LOT to work through. It still is!

As we persevere we are feeling refreshed that spring is upon us again and that we are one year away from that traumatic time of closing our doors with uncertainty of what was to come. With all the changes from 2020, one thing holds true: this team desires to continue enhancing the eyecare and eyewear experience, for you. To support you to see and perform your best in this age of digital connection, helping to build comfort and ease in your world. As a local business, your support, referrals and return to us for your eyecare means the world to us.

Without YOU, we wouldn’t have survived the last year.

Seven weeks.

That was the duration of time we closed our doors to our patients and community when the pandemic hit last March. It changed our way of practicing eye care indefinitely, our systems shifted drastically as the shut-downs impacted every one of us. It was unknown and straight-up scary.

We asked ourselves ad nauseam, “How do we do this? Who do we call for more information? What happens if…? What does that symptom mean? Is it COVID? What processes are in place to ensure our team and patients stay healthy and safe?” Every day was an adventure as we remained steadfast and loyal to “get it right” as best we could.

Procedures We Are Following To Keep YOU Safe!

 We are open for routine care! We have remained diligent in our safety procedures during the pandemic. Here are the top 7 safety measures we practice every day to keep you safe!

1. Masks, of course! We’re all wearing masks, and you are too!

2. Hand Sanitizer and Hand Washing: All team members are consistently sanitizing and washing hands, and we ask that you do the same when entering the office.

3. We’re crazy about sanitizing! All surfaces are sanitized after each patient and throughout the day.

4. Social distance please: We ask all patients to respect a 6ft social distance with other patients, at all times in the office. Orange markers are placed around the office to indicate 6ft between each person.

5. COVID-10 Health Screening Process: Before a patient enters the office, team members are asking a COVID-19 screening question during our check-in process. As patients enter, their temperature is taken to ensure everyone’s safety.

6. By Appointment Only: Only patients with appointments are allowed in the building to help maintain our safety measures.

7. Disinfecting Frames: We sanitize all frames that are tried on during a patient’s visit with our Optical Experts to purchase new glasses.

Make A Difference Daily; Recycle Your Contact Lenses!

Did you know your contact lenses are recyclable?

The majority of contact lens wearers are misinformed about recycling their contact lenses. Every year up to 3 billion contacts end up in wastewater treatment plants due to contact lens wearers tossing their contacts down the drain of their sink. 

Recycling something as small as a contact lens can make a huge impact. Contact lenses cannot be recycled through standard recycling practices due to their small size. They normally get filtered out and end up in landfills. Microplastics, like contact lenses, contribute to the 8 million metric tons of plastic in the ocean. 

 

Together, we can make a difference with recycling your contact lenses, One by One. 

How can you help?

  1. Collect: Hold on to your used foil top, contact lenses, and the blister pack; it is all recyclable!
  2. Drop off your lenses at our office!
  3. Once our recycling container is full, we ship all of the material to TerraCycle. TerraCycle upcycles the material to become park benches, playgrounds, and other structures! 

Any type or brand of contact lenses can be recycled. This program has collected over 6 million items in over two years! This is a simple and easy way to be environmentally conscious, so help us save this beautiful place we call home!

  • Due to COVID-19, the optical labs we use are not operating at full capacity, therefore we have been experiencing longer than normal return times for optical orders. Orders are averaging 3 weeks turn-around. We appreciate your patience for your orders, we will call you once your order has passed our check-in inspection and is ready for you to pick up!

  • Saturday, June 27th is National Sunglasses Day! The number one concern that American adults have about UV eye exposure is vision loss, yet 27% report they do not typically wear sunglasses when they are outside. Remember to take care of your eyes by wearing a quality pair of polarized sunglasses!
